The following part study is adapted from a journal article by Chris Dimick published in June 2008 in the Journal of AHIMA.
The innovation of the EHR has do it easier to reuse previously documented patient entropy. Previous information stop be easily recalled and glued into current keep with a single click. While these shortcuts make certificate easier and faster for engross clinicians, such cut and paste practices can have serious repercussions for patient care and reimbursement, as illustrated in the scenario below.
As the patients health worsened, a infirmary pinch of days turned to weeks. But through the non-homogeneous tests and physician visits, the progress notes generated in the hospitals electronic record system looked similar. Past notes were being electronically copied and pasted into current records. Old services rendered were being documented everywhere and over again, with additions to reflect any new services. This pulling out front of information from past visits, was meant to save time on the busy hospital floor. It resulted in misrepresentation of the patients stay and a fraudulent submission for reimbursement (Dimick 2008a, 40).
